The worst is when a manager turns an estimate into a deadline, just for the sake of having a deadline, even though sales/marketing hasn't asked for one.

Admit it. It's happened to you too.

So true. The vast majority of people I have worked for (and with) will probably never be able to get it.

If they did get it, the systems they have built can not allow quality work, valid estimates, or the ability to communicate to client base on the situation.

An estimate must come always with some uncertainty factor: When asked for estimates, I usually say something like: "there's a 75% chance that this task will take 2 weeks". The actual percentage is less important than the fact that is just a probability.
I get peeved when you give an estimate and "magically" it becomes a deadline... that's why I try to be very careful when asked for it

A nice, measured discussion of the problem - though it doesn't mention the 'Software Death March' outcome explicitly, and I think that happens more than most of us would like to admit. Agree, the McConnell book is great. I actually work on software that attacks the problem of setting realistic expectations based on historical data - here's Joel talking about it: http://www.joelonsoftware.com/items/2007/10/26.html
Sorry about the plug -- this is actually what I work on all day. Anyway, as you point out here, good estimation is only part of it, and communication around deadlines can be terribly difficult when ego and the desire for consistency enter the game.
